Provisional Lorry Licence | Truck School Swindon

Deciding that you want to be an HGV driver is just the first step. You need to study and train for it to be a reality and to do that, you need to have a provisional lorry licence first. If you want to apply for a provisional lorry licence, here’s what you need to do:

Fill out the Provisional Lorry Licence forms.

There are two forms you will need to complete: the D2 and D4.

The D2 form is the actual application form sent by students—this is used to approve the provisional licence. You can order the D2 form pack online from the DVLA, with or without the D4 form. The pack provides instructions for application and other required documentation.

The second form is the D4 or the medical form. A medical examination is required in order to obtain a provisional lorry licence. You can get this with the D2 form pack or separately from gov.uk website. The form is filled out by a doctor—either by your GP or a firm that specialises in medical exams.

After completing the forms, send them to the DVLA.

Both completed forms will need to be sent to the DVLA, along with your photocard driving licence. The application is free.

You will need to include a passport-style colour photo and identity documents such as a current passport, a biometric residence permit, or a travel document if you have a paper driving licence.

You can send these documents to this address:

DVLA
Swansea
SA99 1BR

How long does the process take?

Now that you’ve sent the necessary documents, you will need to wait. You should get your provisional licence within 3 weeks of DVLA getting your application. It may take longer than that if they need to check your personal or health details.
